245D Waivered Services

Armatage Health LLC is a Comprehensive licensed home care agency with the HCBS license designation. Armatage offers the following select Waiver Services: Individual Home Supports (IHS) without training, homemaker, night supervision, Individual Community Living Support (ICLS) services, and adult companion services. For each waiver service, clients select a trusted, qualified worker suited for their needs. In all services, an assigned 245D Service Coordinator will work with the client and county case manager to set up and administer Waiver Services.

245D Waivered Services

Individualized Home Supports Without Training

Individualized Home Supports Without Training services provide support, assistance and supervision to adults who live in their own homes. A staff person provides direct supervision, cueing, guidance, instruction, assistance with activities of daily living, or assistance with coordinating and attending community living activities. Individualized Home Supports Without Training services can be provided in the person’s own home or community.

Individualized Home Supports Without Training was created through the State of Minnesota Waiver Reimagine project, and it combines the previous services of Personal Support and Adult Companion.


Individualized Home Supports Without Training services are available to people on the:

  • Brain Injury (BI) Waiver
  • Community Access for Disability Inclusion (CADI) Waiver
  • Community Alternative Care (CAC) Waiver
  • Developmental Disabilities (DD) Waiver



INDIVIDUALIZED HOME SUPPORTS WITHOUT TRAINING FAQ

While an Individualized Home Supports staff can provide all the same care as a PCA, they are also focused on helping the client to increase their independence and access their community. 


 Yes. Parents can work as an Individualized Home Supports Without Training staff, as long as the client is an adult. 


245D Waivered Services

Homemaker Services

Homemaker Services range from light household cleaning and laundry to assistance with home management and activities of daily living. Home Management includes:

  • Meal prep
  • Shopping for food, clothes and supplies
  • Simple household repairs
  • Arranging for transportation

Homemaker Services are available under various payment options, including both Medical Assistance Home and Community Based Services (waiver programs) and private pay.


245D Waivered Services

Night Supervision Services

Night Supervision is a non-preventative service consisting of overnight assistance and monitoring provided by a staff in the client’s home when the client has an assessed need in one of the following covered service areas:

  • Assistance with activities of daily living (ADLs)
  • Assistance with instrumental activities of daily living (IADLs)
  • Implementation of positive support programming and transition plans
  • Reinforcement of skill development supports


Night supervision services faq

 Night Supervision staff do not need to be awake if the client is sleeping. However, caregivers who live in the same home as the client may not be paid for any time the client is asleep. 


245D Waivered Services

Individual Community Living Supports (ICLS)

Individual Community Living Supports (ICLS) offers assistance and support for older adults who need reminders, cues, intermittent/moderate supervision, or physical assistance to help them remain in their own homes. Individual Community Living Supports covers assistance and support for eligible people 65 years and older in the following service categories:

  • Active cognitive support
  • Adaptive support service
  • Activities of daily living (ADLs)
  • Household management
  • Health, safety and wellness
  • Community engagement


Individual Community Living Supports (ICLS) services are available to people on the:

  •  Elderly Waiver (EW)
  • Alternative Care (AC) Waiver

Your county case manager will supply Armatage with a State of Minnesota Individual Community Living Support (ICLS) Planning Form. The planning form indicates which ICLS Service Categories the individual requires assistance in. Armatage uses this planning form to develop, with the individual, their Coordinated Services and Support Plan (CSSP) Addendum. 


Relatives and non-professional legal guardians/conservators are allowed to provide ICLS services. However, the ICLS Direct Support Staff may not be the person’s spouse, professional legal guardian/conservator, landlord, or have any financial interest in the person’s housing. 


Armatage ICLS Direct Support Staff are able to perform all supports listed in the six ICLS Service Categories, with the exception of medication set-up and administration. Armatage ICLS Direct Support Staff are trained in medication assistance.  


245D Waivered Services

Adult Companion Services

Adult companion services provide non-medical care, supervision and socialization provided to a person age 18 or older. Adult companion services cover services that help a person work toward a therapeutic or community integration goal in their support plan. Here are a few examples:

 

  • Attend a movie with the person to practice coping skills to manage their social anxiety
  • Go with the person to a community event to reduce their social isolation
  • Play a board game with the person to enhance their fine motor skills
  • Provide verbal instructions or cues to the person to help them complete a task
  • Assist or supervise the person with tasks such as laundry, light housekeeping, meal preparation and shopping (only while the companion helps the person work toward a therapeutic or community integration goal)


Adult Companion Services are available to people on the:

  •  Elderly Waiver (EW)
  • Alternative Care (AC) Waiver
